Cyanobacteria, siphon it off. BBA can be killed with hydrogen peroxide then siphoned off.

Too much light, not enough light. Too much fertiliser, not enough fertiliser. Too much flow, not enough flow. Too much filtration, not enough filtration. Take your pick.
