Hygrophila pinnatifida: The flourish and Melting?

I had this plant for about 2 months. It grew from medium size to over 30cm and was stunning! Each leaf turned pink and again it's "looked" amazing and healthy.

And then it just melted. Stems turning to mush and I had no idea what was killing the plant. Nothing had changed EI dosing remained consistent water params remained the same in an established tank. No algae except diatoms from an upping of the lighting.

Any ideas as to what caused this melt?

I remember reading a thread on this forum where UG carpet melt was attributed to a macro fert but I don't remember the thread but I suspect a fert could be the cause. But any advice from you guys would be so greatly appreciated.
 
Really. It's that simple? The skeptic in me thinks that it OD'd on a specific fert but I really and truly appreciate the info.
