Not much PS work at all. Most of the work is done at the capture stage.

This is one exposure and I balanced the sky and foreground by stacking two ND grad filters.

As these selectively block light I could get the extended shutter time, resulting in the smoky water.

If you didn't have filters you could balance the foreground and sky by taking multiple exposures and combining them using software. To get the long shutter speed you would have to use a really small aperture (f/22+) but this can result in compromising image quality and sharpness due to diffraction.

So what I'm saying it's generally best to use filters in this sort of situation.
