What is wrong with my oto?

All are fine except this one had them for a few weeks and just noticed him swimming around erratically on surface and pulling himself out of the water, has one very red eye and slightly red underside around his head? All other livestock are healthy

The zipping around at the surface eating the biofilm is perfectly normal. The redness doesn't look normal but sorry can't help no idea why or what.
 
He’s just died... I’ll post pics on imgur link as I know some don’t like seeing dead things but I want to know what caused it possibly... I know no one is a stranger to a dead oto but mine had all settled
