31ppm of K and Getting Pinholes

Pinholes by defeciency looks more like that :

I would assume what caused that for me was lack of light and maybe flow since this happen when my tank was overgrowth.

Oh boy, now my Hygrophila Serpyllum. Maybe I introduced some aggressive pond snails. I do have a single SAE, Clown Plecos and Oto's.

I suspect this one. Your latest pics look very much like damage from amano shrimp or possibly SAE.
Pleco, oto or snail damage shows up more rasp-like.

Did you substantially increase feeding? Any vegetables?

Just algae wafers. I've tried Zucchini, but they don't eat it.

I would expect SAEs to eat the leaves nibbling from the borders, not attacking at the center.

One of those, Clown Plec would be my guess. That is definitely grazing damage.

Just algae wafers. I've tried Zucchini, but they don't eat it.

I'm assuming <"they've got some wood">? If they have, you could try Yam or Sweet Potato?
