Mechanics of re-planting

I'm planning on taking out my Dwarf Hair Grass (it hasn't done particularly well and looks a little untidy) and Wisteria (it was only ever intended to help get the tank up and running) thought I've not yet decided what I'm going to replace them with.

Draining the tank isn't a real option, so I'm looking for tips on how to replant in a flooded tank with minimum disturbance.

I've replanted my H.c carpet and other bits at water change time, less water in the tank to make planting easier and less risk of spills!
